HPV invades the cell nucleus, causing rapid cell division, accompanied by the reproduction and spread of viral particles, forming characteristic papilloma. The number of concave cells in the granular layer of the epidermis increases, and histologically normal epithelial cells also have HPV. Residual DNA after treatment can often lead to recurrence of the disease.

Sometimes small condyloma acuminatum may cause pain, itching and discomfort in the vulva, and the patient may have hematuria and difficulty urinating. Condyloma acuminatum in the rectum may cause pain and bloody stools, while large condyloma acuminatum in the rectum may cause a feeling of tenesmus.

In male patients, the disease is more common in the frenulum, coronal sulcus, foreskin, urethra, penis, around the anus and scrotum. At the beginning of the disease, there are light pink or dirty red millet-sized growths that are soft in nature and slightly pointed at the top. They gradually grow larger or increase in number. It can develop into a papillary or cystic shape, with a slightly wider base or bands and granules on the surface. The anus is often enlarged and cauliflower-shaped, with a moist or bleeding surface. Pus often accumulates between the grains, emitting a foul odor. Scratching may lead to secondary infection. Genital warts located in drier areas with lower humidity are often small lesions that appear as flat warts. Warts located in hot, humid areas often appear filamentous or papilloma-like and easily merge into large masses. Warts may enlarge in people with severe liver disease. Pregnancy can cause warts to recur or grow faster.

Subclinical infection refers to lesions that cannot be identified by the naked eye clinically, but topical application or wet compress of 3%-5% acetic acid solution for 5-10 minutes can cause whitening in the HPV-infected area, the so-called "acetic acid whitening phenomenon."
