What causes low cardiac enzymes?

The heart is the source of a person's life and is responsible for the power of the blood circulation system. Myocardial enzyme is a value that measures heart function. A value that is too high or too low is not good and may cause some adverse reactions in the body. Some people have high myocardial enzymes, which may be caused by heart failure, myocarditis and other diseases, and people need to pay attention to it. So what causes low myocardial enzymes?

Myocardial enzyme is an indicator of myocardial function. Its increase or decrease can only be explained in combination with clinical findings. It is recommended to make a judgment based on specific symptoms. Myocardial enzymes are mainly indicators of whether myocardial cells are damaged. When the myocardium is damaged or necrotic, these enzymes increase to varying degrees.

It's a problem if myocardial enzymes are high, but it's nothing to worry about if myocardial enzymes are low. Cardiac enzymes are a group of substances released during the destruction of myocardial cells and other tissue cells.

High myocardial enzyme levels indicate damage to myocardial cells, such as myocardial infarction, myocarditis, etc. A decrease generally means that everything is fine and there are no special circumstances.

Myocardial enzymes refer to enzyme substances within myocardial cells, which catalyze myocardial cell metabolism and regulate myocardial cell electrical activity. If myocardial cells become necrotic or ruptured, myocardial enzymes will be released into the blood. Therefore, clinically, the level of myocardial enzymes is generally used to indirectly measure the degree of damage to myocardial cells.

Currently, serum AST, LDH, CK, CK-MB, and α-HBDH are commonly used clinically as relevant detection indicators of hypoxic-ischemic myocardial damage. Among them, AST and LDH are widely distributed in the body and have poor specificity, and are mostly found in kidney and skeletal muscle. CK is abundant in skeletal muscle, followed by myocardium and brain tissue, while CK-MB is mainly present in the cytoplasm of myocardial cells and its content in tissues other than myocardium is very low. It is a myocardial-specific enzyme. α-HBDH is an important enzyme in the process of ketone body oxidation and utilization, and is mainly found in the brain and myocardium. Elevated levels of this enzyme indicate damage to the brain and myocardium, especially when increased levels of α-HBDH are greater than increased levels of LDH, indicating significant damage to the heart and brain.

The incidence of myocardial damage in this group was 80% (66/82), with clinical manifestations of increased or decreased heart rate, low and dull heart sounds, and irregular heart rhythm. After admission, the myocardial enzyme spectrum was elevated. The more severe the asphyxia, the more obvious the increase in the myocardial enzyme spectrum, especially CK-MB, which is the most meaningful. CK-MB best represents myocardial damage in the myocardial enzyme spectrum. Clinical manifestations include increased or decreased heart rate, low and dull heart sounds, and irregular heartbeat. After admission, myocardial enzyme levels were all elevated. The more severe the asphyxia, the more obvious the increase in myocardial enzyme levels, especially CK-MB, which is the most significant. CK-MB is the most representative of myocardial damage in the myocardial enzyme spectrum〔3〕. The present data showed that increased CK-MB was related to the degree of asphyxia. Therefore, quantitative determination of myocardial enzyme spectrum and its isoenzyme activity can be used as an important early, sensitive and specific diagnostic indicator for post-asphyxia myocardial damage.
