1. Basic Content

Through the treatment of spinal (fixed-point) rotation and reduction techniques, the intervertebral disc, annulus fibrosus and intervertebral ligaments of the affected vertebra can be promoted to rotate and pull, thereby generating peripheral pressure on the protruding nucleus pulposus and making the protrusion easier to retract; by correcting the deviated spinous process, the vertebral joints can restore their normal (or compensatory) anatomical position, adapting them to the surrounding muscle groups (i.e. what ancient medical books called "bone closure" and "tendon groove"), relieving the compression of the joint capsule and yellow ligament on the nerve roots, and improving the blood flow of the vertebral artery. In addition, applying rotation techniques to patients with small joint stiffness can also loosen adhesions, increase the range of motion, and relieve pain.

When applying this therapy, the operator should first touch the patient's spine with his fingers to check whether the position of the spinous processes of the relevant vertebrae is normal, whether there is tenderness beside the spine of the affected vertebra, whether the paravertebral muscles are thickened, contracted, or detached, etc., and then use the corresponding reduction technique for treatment.

For example, in the examination and diagnosis of intervertebral disc herniation, Feng Tianyou proposed the following four characteristics: (1) The spinous process of the affected vertebra is distorted. When the doctor palpates the spine with his thumb, he can detect a series of signs of deviated spinous processes: (2) the upper and lower interspinal spaces of the affected vertebra are one wide and one narrow; (3) there is tenderness beside the spinous process of the affected vertebra, or accompanied by radiating pain to the lower limbs; (4) there is cord-like detachment of the supraspinal ligament at the affected site, which is blunt and thick to the touch and has obvious tenderness. The diagnosis can be confirmed if one or two of these clinical characteristics are present.

2. Indications

This therapy has a good effect on traumatic spinal diseases, such as cervical spondylosis, lumbar disc herniation, and certain traumatic paraplegia. Some patients can even see immediate results. In addition, certain therapeutic effects can be achieved during the restoration process for diseases such as hypertension, arrhythmia, post-traumatic brain syndrome, decreased vision or blindness, and deafness caused by spinal diseases.

For patients with cervical spondylosis, post-traumatic dizziness, post-traumatic brain syndrome, deafness, shoulder and arm pain, carbuncle and numbness, etc., with symptoms mainly in the head, face, neck and arms, the cervical vertebrae should be examined and the location of the diseased vertebra should be determined, and the corresponding reduction techniques should be applied.

For patients with arrhythmia, epigastric pain, intercostal neuralgia, diarrhea and other symptoms mainly in the chest and abdomen, the thoracic vertebrae should be examined and the location of the diseased vertebra should be determined, and appropriate techniques should be applied.

For patients with low back pain, pain and numbness in the lower limbs, and bowel and urination problems, the examination and reduction techniques should focus on the lumbar spine.

3. Notes

1. When applying this therapy, accurate positioning of the diseased vertebra is a prerequisite for achieving results, and skilled reduction techniques are the key to improving efficacy. Inaccurate or missed positioning of the diseased vertebrae during examination, and incorrect judgment of the direction of the deviated spinous process may render the treatment ineffective and may even aggravate the condition. The reduction technique must be accurate, the force must be gentle, and roughness must be avoided.

2. During treatment, one reduction cannot correct the deviated spinous process, and continuous treatment is not advisable. It can be combined with massage techniques such as tendon separation, kneading, etc. to relieve spasms, and then reduction techniques can be applied. Some patients need to be treated once every few days, and it may take 4 or 5 consecutive treatments to straighten the deviated spinous process. Do not rush for results.

3. When this therapy is used for reduction in the cervical spine, improper technique may stimulate the vertebral artery and cause collapse. Some patients may suffer from iatrogenic spinal injury and lead to serious consequences such as high-level paraplegia.
