Best treatment for osteonecrosis of the talus

Best treatment for osteonecrosis of the talus

The talus is located between the tibia and fibula, the calcaneus and the navicular bone. As an important weight-bearing bone in the human foot, it is extremely important for the movement of the ankle joint. Talus dislocation is very common in clinical practice. Since most of the vascular supply to the talus muscle comes from the anterior and posterior joint capsules and ligament attachments, once the vascular supply is interrupted after a fracture, the rate of necrosis of the talus after reduction is quite high.

1. What is talus necrosis

Talus necrosis and ischemic necrosis are more common. The talus is the only bone in the whole body that has no muscle attachments. When the ankle joint suffers severe injury, the blood supply to the talus can be completely destroyed and ischemic necrosis may occur. Talus necrosis If talus fracture or dislocation is not treated in time, the incidence of ischemic talus necrosis is 50% to 80%.

2. Causes

1. Fractures of the neck and body of the talus: Most of them are caused by falling from a height, with the heel landing on the ground, and the force going down along the tibia. The reaction force goes up from the heel, and the front part of the foot is strongly dorsiflexed, so that the front edge of the lower end of the tibia is inserted between the neck and body of the talus, causing fractures of the talus body or talus neck, the latter being more common. If the foot is forcefully inverted or everted, the talus may fracture or dislocate. After a talar neck fracture, ischemic necrosis of the talus may occur due to circulatory disorders in the talar body.

2. Fracture of the posterior process of the talus: caused by strong plantar flexion of the foot being impacted by the posterior edge of the tibia or the upper edge of the calcaneal tuberosity.

3. Principles of TCM Treatment

Traditional Chinese medicine has the advantages of safety, effectiveness and low toxicity in the treatment of talus necrosis. The pharmacological effects of its single herbs and compound prescriptions are pleiotropic, and there are multiple effective ingredients or parts at the same time. The compound prescriptions formed according to the principle of syndrome differentiation and treatment have chemical reactions among their components and have new substances and new functions, which makes the compound prescription have a superior overall regulatory function than single herbs, thereby more effectively correcting various imbalances in the body and laying the foundation for the effective treatment of complex diseases.
