Indications for steroid treatment of hyperthyroidism

Exophthalmos is a common complication of hyperthyroidism. From a clinical perspective, the most common treatment method is hormone therapy, which can alleviate symptoms in the eye sockets, relieve the patient's neural edema, and help shorten the course of treatment. When undergoing hormone therapy, it is also necessary to understand some indications, prevent adverse effects of drug treatment on health, and understand some contraindications.

Indications for hormonal treatment of hyperthyroid exophthalmos

It is generally believed that the indications for hormone therapy include: patients with acute inflammation in the eyes, such as obvious protrusion of the eyeball accompanied by conjunctival congestion, edema or inflammatory irritation symptoms; high orbital pressure resulting in corneal lesions or compressive optic neuropathy, and unsuitable for surgery; accompanied by thyroid dysfunction, the cooperation of eye symptoms during endocrine therapy; preoperative medication for orbital decompression; orbital radiotherapy or adjuvant treatment after orbital decompression surgery.

Contraindications of hormonal treatment for hyperthyroidism exophthalmos

At present, the most commonly used hormone type in the clinic for the treatment of exophthalmos caused by hyperthyroidism is glucocorticoids. Glucocorticoids are generally contraindicated in the following situations: ① hyperadrenocortical disease; ② when there is no effective causal treatment for the infection, such as chickenpox and fungal infection; ③ viral infection, such as herpes simplex keratitis, corneal ulcers and vaccinia vaccination;

④ Active peptic ulcer; ⑤ Recent gastrointestinal anastomosis, fracture, or trauma repair; ⑥ Moderate or above diabetes; ⑦ Severe hypertension (except for those caused by systemic lupus erythematosus, etc.); ⑧ Early pregnancy and postpartum period.
