These diseases are called precancerous diseases because people with this disease have a higher chance of developing gastric cancer than other people. However, it must be mentioned that having these precancerous diseases does not mean that they will all develop into cancer in the future. Only some cases will develop into cancer. The following are several precancerous diseases that are currently receiving attention from clinical workers. 1.1 Chronic atrophic gastritis Animal experiments have proven that nitrosamines can indeed cause cancer. In chronic atrophic gastritis, low gastric acid is conducive to the growth of nitrate-reducing bacteria, which makes the widely present in food Nitrates are converted back to nitrous acid, which combines with amines to form carcinogenic nitrosamines. Especially for chronic atrophic antral gastritis, the possibility of developing into gastric cancer is greater, but it takes longer. Some people have followed up and observed that about 10% of patients with chronic atrophic gastritis develop gastric cancer, especially atrophic antral gastritis accompanied by intestinal metaplasia and atypical hyperplasia, which are prone to cancer. It is generally believed that intestinal metaplasia type II or incomplete type, which resembles colon mucosa and goblet cells, secretes sulfated mucin and is prone to cancer. The course of chronic atrophic gastritis is prolonged, and some patients experience symptoms of indigestion, namely upper abdominal fullness and discomfort (especially after meals), irregular abdominal pain, belching, acid reflux, nausea and vomiting, etc. These symptoms are not specific. Most of them have no obvious symptoms, but sometimes there may be mild tenderness in the upper abdomen. Severe gastritis in the gastric body may cause glossitis and anemia. 1.2 Gastric polyps There are two types of gastric polyps: hyperplastic and adenomatous. The latter is rare. According to relevant statistics, its incidence was 0.29% in 8,735 autopsies, and only 1 case was found in 8,000 gastric surgeries. Gastric adenomatous polyps are prone to malignant transformation. According to literature statistics, about 20% of adenomatous polyps turn into gastric cancer, especially multiple polyps larger than 2 cm. Increased numbers of cancer cells are often considered to be precancerous diseases. Polyps usually occur at an older age, most commonly between 55 and 75 years old, and are more common in men. Early symptoms are often mild pain and discomfort in the upper abdomen. If a larger polyp blocks the pyloric duct or slides into the duodenum, symptoms of pyloric obstruction will appear. Because such patients often have a lack of or low gastric acid, nausea, anorexia, indigestion, weight loss, diarrhea, etc. are often more obvious. If there is erosion or ulcer on the surface of the polyp, intermittent bleeding may occur. 1.3 Gastric ulcer Most clinical and pathological workers believe that some gastric cancers may develop from the malignant transformation of gastric ulcers, but the incidence is low. According to reports from some pathology departments in Shanghai and Beijing, the incidence of gastric cancer caused by the malignant transformation of gastric ulcers is 1% to 5%. Cancer occurs at the edge of the ulcer. For patients with chronic gastric ulcer who are over 45 years old and whose symptoms become stubborn and have not been effectively treated with strict medical treatment for about a month, and whose stool occult blood test turns positive at the same time, the possibility of canceration of the ulcer should be considered, and further examination and multiple biopsies at the edge of the ulcer are needed to clarify the nature of the ulcer. 1.4 Remnant stomach The cancer rate of the residual stomach after gastrectomy is twice as high as that of the normal population, and it usually takes 15 to 30 years. This is because the bile reflux after surgery often causes atrophic gastritis, which forms the basis for cancer. In addition, abnormal cell reproduction due to acid deficiency promotes the conversion of carcinogens such as secondary amines or nitrites into nitrosamines. The bile salts in the bile themselves also have carcinogenic effects. Therefore, the residual stomach is also considered a precancerous disease. 1.5 Pernicious anemia It can lead to atrophic gastritis, among which atrophic gastritis of the gastric body has a certain relationship with the incidence of gastric cancer. Some people regularly observed 211 patients with pernicious anemia and found that the incidence of gastric cancer was as high as 8%. There are also reports that the incidence of gastric cancer in this disease is 4 times higher than that of people of the same age without this disease. However, there are relatively few patients with pernicious anemia in my country, so it does not constitute the cause of gastric cancer in Chinese people. |
