3. Abnormal position of the uterus: When the uterus is extremely retroflexed or anterior, an angle may be formed at the isthmus of the uterus, which may obstruct the flow of menstrual blood. The menstrual blood may stimulate the spasmodic contraction of the uterine muscles and cause premenstrual abdominal pain.
