Lung abscess usually has an acute or subacute onset, initially with chills, high fever, cough, and coughing up mucus or mucopurulent sputum. If the inflammation spreads to the pleura, there will be chest pain. If the lesion is widespread, the poisoning symptoms are severe, with general weakness, shortness of breath, rapid heartbeat, sweating, and loss of appetite. After 1 to 2 weeks, the abscess breaks into the bronchus, and a large amount of purulent sputum is suddenly coughed up, up to several hundred milliliters a day. Due to the anaerobic infection, the sputum has a foul odor and is divided into three layers after standing, consisting of foam, mucus and pus residue from top to bottom. After the pus is discharged, systemic symptoms improve and the body temperature drops. If effective antibiotics are used in time, the lesions can gradually improve within a few weeks, the body temperature returns to normal, the sputum volume decreases, and the general condition returns to normal.

If treatment is not timely and thorough, medication is inappropriate or insufficient, and the body's resistance is low, the disease may gradually become chronic. Some break into the chest cavity to form purulent pneumothorax or bronchopleural fistula. At this time, the symptoms are sometimes mild and sometimes severe, mainly coughing and coughing up purulent sputum. Many people also have hemoptysis, ranging from blood in sputum to severe hemoptysis, intermittent fever and chest pain, etc. Due to long-term chronic poisoning and consumption, many patients become emaciated and anemic. Some patients have metastatic abscesses in the brain, liver, and kidney. Chronic abscesses often have a history of irregular treatment, with slight improvement when the lesion stabilizes.

Signs: Early lesions are small and have no special signs. Signs of lung consolidation (such as decreased breath sounds, dullness to percussion, bronchial breath sounds, and inspiratory crackles) as well as pleural friction rubs, pleural effusions, pulmonary edema, and pyopneumothorax (dullness to percussion, contralateral shift of the mediastinum, and decreased breath sounds at the site of the effusion) may be found. Urn-like breath sounds are rare. Patients with a longer course of the disease often have clubbing of the fingers, collapsed and deformed chest cage, and poor mobility. Examination of patients with pneumothorax and bronchopleural fistula may reveal corresponding physical signs.
