Treatment of anaphylactic shock

There are many reasons for shock. Some people suffer from physical weakness, some suffer from hypoglycemia, and some suffer from allergies. But no matter what the reason is, when we think someone faints, we think something big has happened, and it is such a terrible thing. There are many types of allergies. Severe allergies can cause shock and death. So we need to learn some practices to help them.

If a patient suffers from drug-induced anaphylactic shock, stop taking the medication immediately, provide first aid on the spot, report to a doctor promptly, and proceed as follows.

1. Lie down immediately and inject 1 mg of epinephrine subcutaneously as directed by the doctor. For children, reduce the dose appropriately and keep warm.

2. Give oxygen inhalation. In case of respiratory depression, artificial respiration should be given as ordered by the doctor and tracheotomy should be performed if necessary.

3. In case of cardiac arrest, immediately perform cardiopulmonary resuscitation and other rescue measures.

4. Rapidly establish intravenous access to replenish blood volume.

5. Closely observe the patient's consciousness, vital signs, urine output, and other clinical changes.

6. Accurately record the rescue process.

Western medicine treatment of anaphylactic shock

We must act decisively and deal with it proactively without losing any time.

1. Immediately stop entering and remove suspected allergens or pathogenic drugs. Ligature the limbs above the injection or insect bite site to slow down absorption. The injected or stung area can also be blocked with 2 to 5 ml of 0.005% epinephrine.

2. Immediately administer 0.1% epinephrine, first inject 0.3-0.5 ml subcutaneously, then inject 0.1-0.2 ml through venous puncture, followed by drip of 5% glucose solution to maintain smooth intravenous administration. Epinephrine can rapidly relax bronchospasm through its β-receptor effect and constrict peripheral small blood vessels through its α-receptor effect. It can also counteract the release of some mediators of type I allergic reactions, so it is the first choice drug for treating this disease and can be used repeatedly several times during the course of the disease. Generally, after 1 to 2 injections of epinephrine, most patients' shock symptoms can gradually recover within half an hour. On the contrary, if the shock persists and does not improve, it is a serious case and 10-20 mg of dexamethasone and 200-400 mg of hydrocortisone succinate should be injected intravenously as soon as possible. A number of anti-shock drugs with longer-lasting efficacy and fewer side effects, such as norepinephrine, alamin (metahydroxyamine), etc., may also be used as appropriate. At the same time, vasoactive drugs are given and blood volume is replenished in time. The first dose of 500 ml of fluid can be quickly dripped in, and the fluid volume for adults on the first day can generally reach 400 ml.

3. For anti-allergic and symptomatic treatment, commonly used drugs are 10 mg of chlorpheniramine or 25-50 mg of promethazine, which are injected intramuscularly. The patient should lie flat and inhale oxygen to keep the airway open. Because the patient's allergic threshold is very low when suffering from anaphylactic shock, some drugs that were not originally allergenic may become allergens. Therefore, excessive use of medication should be avoided when treating this disease.

TCM treatment of anaphylactic shock

Yin Exhaustion and Yang Deficiency Syndrome

Symptoms: After using drugs such as penicillin and streptomycin, there may be sudden chest tightness, palpitations, difficulty breathing, coma, convulsions, profuse sweating, or sweating like beads or oil, or even weak breathing, cold limbs, and a weak pulse.

Treatment method: Consolidate Yin and restore Yang.

Main recipe: Shenfu Decoction

Usage: Immediately intravenously inject Shenfu injection or Shengmai injection, followed by taking Shenfu decoction.

Add or subtract: If there is excessive sweating, add Cornus officinalis; if there is convulsion, add cicada shell, scorpion, and Schizonepeta tenuifolia.

Wind poison severe syndrome

Symptoms: After using drugs such as penicillin and streptomycin, there is sudden chest tightness, palpitations, difficulty breathing, itchy skin, urticaria, and even coma, convulsions, and a floating and stringy pulse.

Treatment method: Soothe wind and chest poison.

Main recipe: Xuandu Fafang Decoction

Add or subtract: For convulsions, add Bombyx Batryticatus and Uncaria rhynchophyl

Other treatments:

Wind syncope is an acute and critical illness and rescue should be carried out as quickly as possible. Any delay may put the patient's life in danger. The patient should lie flat without a pillow, be given oxygen, and keep the airway open. In an emergency, intubation and oxygen administration, or tracheotomy should be performed.

Immediately inject 0.5-1 mL of 0.1% epinephrine subcutaneously and inject 10 mg of dexamethasone intravenously. Emergency treatment such as promethazine and 10% calcium gluconate injection can also be used.

Acupuncture therapy: urgent puncture of Ren Zhong, Yongquan, Baihui and other acupoints; or heating of Baihui, Qihai, Shenque; or bloodletting of Shixuan acupoint.

In the future, the use of drugs or foods that cause this disease, such as penicillin, streptomycin, immune serum, or eating shrimp, should be banned.

Emergency Chinese patent medicine: Add 10% glucose injection to 10-20mL of ginseng and aconite injection and immediately push it intravenously, or add 20mL of 10% glucose injection to 10-20mL of Shengmai injection and immediately push it intravenously.
