Why is my whole body itchy like being pricked by needles?

1. Generalized pruritus is often a concomitant or initial symptom of many systemic diseases, such as uremia, biliary cirrhosis, hyperthyroidism or hypothyroidism, diabetes, malignant tumors and neuropsychiatric pruritus. The external causes of systemic pruritus are related to environmental factors (including humidity, season, biological or chemical stimulation in the working environment), topical medications, the use of highly alkaline soaps, and the decreased secretion function of the patient's sebaceous glands and sweat glands leading to dry skin. 2. The causes of localized pruritus are sometimes the same as those of systemic pruritus, such as diabetes. Anal pruritus is often related to pinworm disease, hemorrhoids, anal fistula, etc. Vulvar pruritus is often related to leucorrhea, trichomoniasis, vaginal fungal disease, gonorrhea and cervical cancer. Scrotal pruritus is often associated with local high skin temperature, sweating, friction, and fungal infection. The occurrence of itching is mainly caused by the release of chemical mediators such as histamine, substance P, kinins and proteases.

Clinical manifestations 1. Systemic pruritus is more common in adults. The itching often starts from one place and gradually spreads to the whole body. It is often paroxysmal, especially at night. In severe cases, it presents as persistent itching with paroxysmal aggravation. Drinking, coffee, tea, mood changes, spicy food stimulation, mechanical scratching, warm bedding, and even some suggestion can induce the onset and aggravation of itching. Scratches, blood crusts, pigmentation, and even eczematous changes, lichenoid changes, pyoderma, lymphangitis, and lymphadenitis are often secondary. (1) Senile pruritus is common in the elderly, with the trunk being the most itchy. It is mostly caused by factors such as sebaceous gland dysfunction and dry skin. In female patients, it may be a manifestation of postmenopausal syndrome. (2) Winter pruritus is more common in adults, but children can also suffer from it. It often occurs in late autumn and winter when the temperature changes dramatically. Patients often begin to itch when they enter a warm room or take off their clothes before going to bed. (3) Summer pruritus is often caused by dampness and heat. Increased sweating in summer can aggravate the itching.

2. Localized pruritus (1) Anal pruritus is more common in middle-aged men, and children with pinworm infection may also suffer from the disease. The itching is usually confined to the anus and the surrounding skin, but can sometimes spread to the perineum, vulva, and scrotum. Frequent scratching may cause the anal skin to thicken, but it may also present as secondary damage such as lichenification or eczema-like changes. (2) Scrotal pruritus: The pruritus is mainly confined to the scrotum, but may sometimes also affect the penis, perineum and anus. Due to constant scratching, lichenification, eczematous changes and secondary infection may occur. (3) Vulvar pruritus: Itching often occurs on the labia majora and minora. Due to constant scratching, the labia majora often have thickening and infiltration of the skin, and the clitoris and vaginal mucosa may have redness, swelling and erosion.
