1. Treatment principles for retinal vein occlusion

(1) Central retinal vein occlusion: Iris neovascularization needs to be closely monitored. Once detected, panretinal photocoagulation must be performed immediately.

(2) Branch retinal vein occlusion: Large areas of non-perfused retina appear, which can be treated by photocoagulation of the non-perfused areas.

(3) Severe macular edema can be treated by macular grid photocoagulation or intravitreal drug injection.

2. Drug treatment of retinal vein occlusion

This disease is difficult to treat and there is controversy over some of the treatments. In theory, thrombosis should be treated with anticoagulants, but in reality the effect is not ideal, and many anticoagulants used in the past are no longer used. So far there is no specific and effective treatment. Generally, treatment and prevention of thrombosis can be targeted at the cause, such as lowering blood pressure and intraocular pressure, reducing blood viscosity, alleviating thrombosis and tissue edema, and promoting bleeding absorption.

3. Laser treatment of retinal vein occlusion

Its mechanism is: ① reduce capillary leakage, form a barrier to prevent fluid from penetrating into the macula; ② seal the non-perfused area to prevent the formation of new blood vessels; ③ seal the new blood vessels to reduce and prevent vitreous hemorrhage. Laser treatment of main trunk obstruction can only prevent neovascularization and reduce macular cystoid edema, and has little effect on improving vision, but it is more effective for branch obstruction.
