Real fire refers to the syndrome of real heat caused by excessive yang heat. Liver, gallbladder, and gastrointestinal fire are the most common. Symptoms include high fever, headache, red eyes, thirst for cold drinks, irritability, abdominal distension and pain, constipation, yellow urine, red tongue with yellow and dry coating or thorns, rapid and strong pulse, and even vomiting blood, nose bleeding, etc.

In treatment, it is advisable to adopt the principles and methods of controlling fire with bitter and cold herbs, clearing away heat and detoxifying, and purging the solid and eliminating fire.
