Orthodontic treatment is a rather long process. Studies have shown that enamel demineralization of a single tooth is prone to occur on the gingival side around the bracket. The occurrence of enamel demineralization is closely related to whether orthodontic patients can effectively remove plaque and maintain good oral hygiene. Therefore, oral health education cannot be ignored. Orthodontists should often verbally praise patients for maintaining good oral hygiene during treatment, thereby motivating patients to cooperate with treatment; at the same time, they should also inform patients of the correct oral cleaning methods. In addition to oral hygiene education, orthodontic patients should also undergo full mouth cleaning regularly.

Use of fluoride

Fluoride can inhibit enamel demineralization by reducing the solubility of enamel and inhibiting the formation of acid and bacterial activity in plaque. Fluorine replaces part of the hydroxyl groups in the hydroxyapatite unit cell to form a fluoroapatite unit cell, filling the vacant ion positions in the hydroxyapatite unit cell, thereby enhancing the crystallinity, stability and hardness of apatite. The products of fluoride action are calcium fluoride and fluorapatite, which promote the remineralization of enamel in demineralized areas. Even trace amounts of fluorine can promote remineralization by accelerating the regrowth of porous crystals. When fluoride is used systemically or topically, the action of fluoride mainly occurs at the interface between plaque and tooth surface or tooth surface and saliva. A large amount of free fluoride ions form a temporary fluoride reservoir there, which greatly promotes the remineralization of enamel. The fluoride ion concentration in plaque and saliva also increases accordingly, prolonging the duration of fluoride action.

It is a common phenomenon that enamel demineralization occurs in orthodontic patients during and after orthodontic treatment, but orthodontists generally ignore the existence of this problem. Enamel demineralization is an early manifestation of dental caries. If it is not prevented, it will eventually cause lifelong damage to the teeth. Therefore, the prevention of enamel demineralization during orthodontic treatment is urgent. In addition to strengthening oral hygiene education for patients and advising them to reduce their carbohydrate intake, preventive measures should also include the use of some fluoride preparations such as fluoride sustained-release adhesives and fluoride toothpaste to reasonably and effectively prevent the occurrence of enamel demineralization during orthodontic treatment.
