Even when the manifestations of acute severe cholangitis such as mental symptoms and shock appear, the patient may still have no obvious abdominal pain and jaundice. Physical examination may reveal asymmetric enlargement and tenderness of the liver, which is often misdiagnosed as liver abscess or hepatitis. This periodic intermittent attack is the characteristic clinical manifestation of intrahepatic bile duct stones.
